The Program Coordinator for Outreach MUST have strong case management skills to oversee our case management services. The Program Coordinator monitors, supervises, and participates in the day‑to‑day activities of the Outreach Program and the Drop-In Center for youth and young adults who are experiencing homelessness, at high risk of experiencing homelessness, or are experiencing trafficking and exploitation. The Program Coordinator supervises Youth Workers and Relief staff, provides crisis intervention, information and referrals, advocacy, and support services for youth and young adults. Additionally, the Program Coordinator maintains an atmosphere of trust, respect, and equity.

Primary Duties:

Maintains a caseload of case management clients and covers staff absences.
Oversees daily operations and staffing of the Drop-In Center.
Oversees intakes, eligibility, and recordkeeping for youth in the Drop-In Center.
Provides consultation and assistance in crisis interventions.
Assists in collecting and analyzing relevant data for purposes of monitoring the program’s quality and effectiveness.
Develops a good working knowledge of the adolescent service system, public assistance programs, and health/mental health services in the community.
Supervises and evaluates the job performance of Youth Workers and Relief staff.

Requirements

Must have supervisory experience with two years of case management preferred. Bachelor’s degree in human services related field or Bachelor’s degree in a non-human services related field with 2 years of human services experience in order to qualify as a Targeted Case Manager (TCM).
